The Ohio Department of Health is conducting a statewide health and well-being assessment of people with disabilities. One component of the assessment is a survey of Ohioans with disabilities and/or their in-home care providers. Information collected through the survey will inform the development of the State Health Improvement Plan. The anonymous survey may be completed by an individual with a disability or informal care providers of a person with a disability. Caregivers may assist those with a disability in completing the survey. An example of help may be reading the questions aloud and recording their responses. If a person you care for is unable to participate directly in the survey, you can respond as a caregiver on their behalf. There are also questions about informal caregiving. Respondents must be 18 years of age or older to participate. The last date to complete the survey is March 29, 2023.
