- Maintain a regular schedule (round trip) for transporting children and adults from their residences to school, adult services programs, and between programs as well as field trips, basketball games, special events, etc.
- Before starting the daily route, check all lights, emergency equipment, brakes, tires, engine, oil, and fuel tank gauge. When necessary, replenish all fluids to the proper operating level.
- Must assist the transportation aide when needed with handicapped passengers as they ascend or descend the bus or vehicle or to maintain good behavior and/or safety of other riders.
- Must be especially alert in assisting the transportation aide as the mechanical/electrical/hydraulic equipment (lift) is being operated to raise and lower wheelchairs, in the opening and closing of door openings, in the secure placement of wheelchairs using all locking devices and the comfort and safety of passengers. This includes safety belts, car seats, and safety vests with all their locking devices in place and properly fastened.

- Assist in all aspects of personal care that includes, but is not limited to, positioning, feeding, toileting, and clean up of individual that may include food spills, incontinence, vomit and blood spills. Utilize universal precautions procedures.
- Assist with mobility which includes walking, operation of a wheelchair, and transfer of individual from various items of furniture or toilet fixtures, bus loading and unloading.
- Assist in maintaining clean personal space for individuals served by such actions as wiping wheelchair trays clean, ensuring clean work surfaces, etc.
- Perform services in a manner that demonstrates respect for the individual and their needs, wants and preferences utilizing individualized and appropriate strategies and techniques.
- Participate in interdisciplinary team meetings as requested/assigned.
- Maintain accurately all records required by statute, rule or position relative to personal care and mobility assistance performed as part of job tasks.

- Provide clerical support to staff, such as typing, word processing, data entry, and other routine clerical tasks (i.e., faxing, copying, filing, answering phones, processing mail, handling purchase orders for both MASCO and the Board, handling field trip requests, handling petty cash requests, processing emergency reports, etc.).
- Handle correspondence, reports, memos, payroll, letters, or other material of technical and confidential nature. Act as the facility's first line liaison; answering questions and requests in areas not requiring personal attention of supervisor. Provide initial contact with general public, office visitors, administrative staff, etc., screen calls and visitors according to office procedure. Handle communication systems, such as paging, intercom, and use of wireless communication devices.
- Responsible for organization of the filing systems, record keeping methods and office use forms.
- Relieve immediate supervisor of routine administrative duties such as gather statistics and other general information; do research, prepare reports; respond to questions and requests in areas not requiring the personal attention of the supervisor.

- Coordinate intake and eligibility processes to establish an individual's eligibility for county board services. Obtain collateral information for individuals requesting services. Administer Children's/Ohio Eligibility Determination Instrument (C/OEDI) and/or eligibility for children less than six years of age as assigned.
- Perform 24 hour crisis intervention services as scheduled. Carry cell phone at all times during designated rotations. Provide crisis assistance to ensure that needed services are accessed in a critical or unexpected situation.
- Provide crisis intervention, information and referral, and service response to individuals seeking assistance from the county board.
- Assist individuals in accessing all needed/requested services which includes integration of resources directed to meeting the multiple needs of individuals and assuring that they receive and retain the services and/or supports to enable them to meet their goals.
- Ensure the rights of individuals in accordance with federal, state and local statutes & regulations, and to remove the barriers to improve the individual's quality of life.
- Will be considered the Single Point of Accountability for individuals served.
- Assess individual needs for services, coordinate Individual Service Plan (ISP) conferences in keeping with the philosophy of self-determination. Reduce service duplication and plan services to meet an individual's unmet needs through one single unified document. Facilitate the development of the individual service plan and write the ISP. Coordinate needs on level of care assessments. Document service dates, corrections, suspensions and disenrollment. Coordinate changes and make all ISP revisions.
- Assess, write, implement, and monitor behavior support plans in accordance with OAC, ORC, and county board policies and procedures for individuals needing behavioral intervention. Provide consultation, training and assistance to families, providers and community agencies.
- Assure the individual's health and safety needs are met through the development and implementation of the ISP.
- Establish budgets for services based on the individual's assessed needs and preferred ways of meeting those needs.
- Assist individuals in making selections from the providers they have chosen and ensure that services are effectively coordinated and provided by appropriate providers.
- Under direction, establish and implement on-going system of monitoring the implementation of individualized service plans.
- Implement results, QA Reviews and identified patterns and trends into amendments of an individual's service plan. Conduct service monitoring during flexible and variable hours as assigned.
- Assist in the coordination and development of a Life Plan for individuals assigned.
- Assure level of care and/or comprehensive functional assessments are completed. Complete necessary paperwork and apply for waivers for individuals. Assure all protocols are completed for Home and Community Based Service Waivers and Supported Living recipients. Complete PAWS and PAS for individuals.
- Meet in person at least quarterly with all individuals on caseload or as otherwise assigned to determine satisfaction of services and to monitor the ISP.
- Monitor the ISP to review the individual's progress towards the identified goals and objectives in the ISPs.
- Follow up with service providers on 'plans for improvement' for services listed in the ISP in accordance with federal, state and local statutes.
- Complete all documentation (Targeted Case Management, Service Coordination and Waiver Coordination, etc.) in keeping with county board requirements as well as applicable Medicaid requirements.
- Report unusual incidents and major unusual incidents in accordance with House Bill 94/405 and Ohio Revised Code, Ohio Administrative Code, and county board policies and procedures. Complete follow up on major unusual incidents.
- Complete all referrals (inter and intra agency) in a timely manner and provide follow up to ensure linkage to appropriate services/agencies.
- Monitor compliance with court orders for members of department class action suits.
- Process all Pre-Admission Screening Resident Review (PASRR) requests received from the ODDD and District XI Area Agency on Aging according to OAC Rules mandating timelines, protocols for appropriate placement and level of care.
- Initiate and process Individual Information Forms (IIFs) at the initial time of eligibility and any time changes occur for an individual that may affect an outcome on the IIF.
- Make initial recommendations to ODJFS regarding level of care and determine whether the individual meets the criteria for an ICFMR level of care, in accordance with OAC, at the time of initial application for enrollment on an HCBS waiver. Certify the continued need for an ICFMR level of care for the annual redetermination.
- Staff spending most of their time dealing with service and response will be primarily responsible to provide crisis intervention, information and referral, and service response to individuals seeking assistance from the county board.
- Staff spending most of their time dealing with behavior support will be primarily responsible to assess, write, coordinate and implement behavior support plans for individuals needing behavior support intervention. Staff will be a resource to MCBDD staff, families and the community and will provide consultation, training, education, and assistance in behavior support.
- Staff spending most of their time dealing with intake/eligibility will be primarily responsible to implement and coordinate intake and eligibility processes to establish an individual's eligibility for county board services; to obtain collateral information for individuals requesting services; and to administer C/OEDI and eligibility for children less than six years of age as assigned.

- Assist passengers in ascending or descending the bus or vehicle with the use of the mechanical/electrical/hydraulic chair lift and/or the regular door entrance way with steps as needed.
- Assign a safe and comfortable seating arrangement for all passengers to and from home or for any specified trip. The basic procedures for this includes secure placement of wheelchairs, car seats, safety vests, seat belts with all locking devices in place and properly fastened.
- Maintain good behavior and discipline of passengers while en-route for the safe transportation of all passengers. Maintain rider information records and see to the needs of each individual.
- Follow directions of the bus or vehicle operator and assist whenever called upon. Assist bus or vehicle operator with ice/snow removal from bus or vehicle when necessary.
- Maintain daily cleanliness of bus or vehicle interior.

- Assist certified teacher/specialist in the education of children with developmental delays in approved curricular areas. Assist in maintaining a safe, family-friendly environment conducive to learning. Assist in implementing lessons and/or behavior programs consistent with the objectives of the individual education plan. Assist in operating equipment, recording responses, and preparing educational materials. Assist in maintaining records/reports.
- Assist children with physical and personal needs such as positioning, toileting, feeding and individual clean-ups that may include food spills, incontinence, vomit and blood spills. Assist with feeding, mobility, which may include children walking, operation of a wheelchair, transfers, loading and unloading buses, lifts, standers and other adaptive equipment.
- May work with/assist substitutes assigned to their work area.
- An Instructor Assistant assigned to the pool assists the swim instructor in maintaining the health and safety of every child in the pool area, in enforcing rules for safety, in teaching basic swim skills, in assisting children in and out of the pool, and in dressing of children after pool therapies. Will also be responsible for life guarding. Must be aware of children with special problems and be constantly alert for any unusual behaviors that might indicate a child needs attention (i.e., seizures). Must be familiar with safety equipment that is needed for adaptive use.
- An Instructor Assistant assigned to the physical therapy department will assist in maintaining a safe environment conducive to the physical therapy department. Assist in operating equipment, recording responses, scheduling students for physical therapy and physical therapy swim periods, and in transporting students to and from the physical therapy department. Assist therapist in treatment when requested.

- Responsible for the supervision and training of adults with mental retardation or developmental disabilities in habilitation and production activities in a sheltered workshop or other appropriate setting using individualized and appropriate strategies and working as a member of a unit team.
- Responsible for developing and implementing daily/weekly plans for their assigned unit in a team approach. These plans will be based on the annual plan as developed by the Habilitation Specialists responsible for the individuals assigned to the unit and may include specific behavior support techniques to be used for specific individuals assigned to the unit.
- Responsible, as a member of the unit team, for the day to day operation of their assigned unit including providing for the health, safety and welfare of the individuals assigned to that unit. Responsibilities with regard to day to day operation include the physical set up of the unit area; the set up of production activities including the supervision of the work and the provision of quality control; the set up of habilitation activities as included in the plan; clean up of individuals or their personal space as a result of spills or bodily secretions. Display knowledge of and utilize universal precautions procedures in the performance of the latter.
- Participate in unit team meetings, the individuals' annual team meetings as requested and special team meetings as needed. Assist in the development of the individuals' annual plans and behavior support plans.
- Complete required documentation such as that required for, behavior support charting, daily goal implementation sheets and other such items.
- Display the ability to function as a team member, sound judgment, the ability to complete and maintain appropriate documentation and knowledge of appropriate agency policy and procedures.
